Update Manager Question (v. 4.1)

I know, I know... I ask a million questions on these forums.  But! - in my defense, I walked into an environment that has no documentation, no resources and no one with any real knowledge of what's going on. OK? :-)

Now, we want to implement Update Manager into our environment.  I see that there are various updates/releases of it for 4.1.  Is this a situation of having to download the original and then install the others on to of it? Or can you just download and install the latest release?
